Subject: STARLAB equipment maintenance -
Shape-Up
This page provides information about the
STARLAB Shape-up. If any part of your STARLAB needs repairs, inspection,
cleaning or a tune-up, follow these instructions to get ready to ship the
equipment back to the factory. Equipment is taken for repairs on a first-come,
first-served basis. After reviewing your equipment, get it ready for shipment.
Follow these steps:
1. After determining
needed repairs, compare it to the order form.
2. Purchase Orders
authorizing service to the equipment for STARLAB Shape-up should be made to
Learning Technologies, Inc. It should not include any new components. Purchase
orders for new equipment should be made to Kensington House Ltd.
3. Fax the order form
in advance of shipping your equipment, so that LTI can review your needs and
anticipate delivery. Fax 617-628-8606
4. A completed order
form (same as faxed in step 3) and the original institutional purchase order
should be included with the equipment shipped for the STARLAB Shape-up
5. Carefully prepare
your equipment for shipment, paying particular attention to the Additional Notes
section of the attached instructions. Ship to: Learning Technologies, Inc., 43
Elmwood Street, Somerville, MA 02144.
6. Invoicing for
STARLAB Shape-up will come directly from Learning Technologies, Inc. Billing for
repairs and shipping charges will be mailed soon after return shipment.
Orders for new equipment should be made on
separate purchase orders and addressed to Kensington House, Ltd., P. O. Box 215,
Chickamauga, GA 30707 or faxed to 706-375-3919. If you need to discuss the
purchase of new equipment, telephone 1-800-772-2366 or e-mail your question to:
starlab@cdc.net.

information are self-explanatory. Here are the three pages you will need:

2. The Dome Repair Diagram
3. The Shape-Up Order Form
(You can print these pages directly out of
Netscape or Internet Explorer if your page setup uses minimum margins. Also, you
can copy the pages and paste them into your favorite word processing program,
then print them from there.)
The order form
should be faxed to Learning Technologies at 617-628-8606 prior to shipping the
equipment. Equipment should be well packed, correctly labeled and insured. Pay
particular attention to the Additional Notes section of the
instructions.
